Please understand that your contract governing your dental benefits is between you, your employer, and your insurance company. We are not a party to this relationship in any manner. Your obligation to our practice is to pay for the treatment regardless of the amount that may or may not be reimbursed by your insurance company.
In most cases, our office will accept payment directly from your insurance company with the following provisions in mind. We are willing to submit insurance forms and claims on your behalf, provide any documentation to you or your insurance company to sort out any questions or confusion that may arise, and will cooperate with the regulations and requests of your insurance company. Completing insurance forms is a courtesy that we extend to you in an effort to maximize your insurance reimbursement and make your visits as convenient as possible. However, this does not eliminate your financial obligations to us for your treatment.
We will not enter into a dispute with your insurance company over any claim. Again, we will be more than willing to supply you with any information that you may require while disputing a non-payment. But it remains your responsibility to resolve any type of dispute over payments made or not made by your insurance company.
