Your First Visit

When scheduling your first appointment, please notify us regarding any medical condition(s) that may be of concern or any medication(s) you may be taking.  For your convenience, please make a list of these medical conditions or medications and bring them with you to your first appointment. Many people find it much easier to recall this information while at home with records available.

Unless you have a dental emergency or specific concern, your initial appointment will be with one of our experienced dental hygienists and will consist of a dental prophylaxis (cleaning), an oral cancer screening, and a full-mouth series of x-rays. Please be aware that your first visit may vary slightly due to your needs or oral health condition. On your second visit, you will meet your Doctor for a comprehensive examination which will include the review of your x-rays, taking needed intra-oral photographs, completing a full periodontal charting, and discussing your treatment plan.  At this visit, we want you to share any concerns and share any specific wants that you would like to accomplish. With your input, your personalized treatment plan can include required treatment suggestions along with all of your wants. In some cases, such as emergency situations or per your request, treatment can be completed the same day as your comprehensive exam.

Please assist us by providing the following information at the time of your visit:

  • Any current X-rays, if applicable
  • A list of medications you are presently taking
  • Your insurance information, if applicable

To save time, we encourage you to download the following PDF documents print, fill in, sign and bring with you to your first visit:
